If it's not that then it could be the fuel vaporiser in the exhaust pipe; it injects diesel in to the exhaust to raise the temperature to allow a DPF regen, if it fails then the regen fails. I'm in the process of replacing the vaporiser on my car (it's on order), £250 from Ford but £150 on ebay.
